Looks like ya got them Tomato worms there...Check under the leaves for green catapiller looking things.

Looks Like Catfacing to me. It can be much worse, throw dat one away Jeanette. In some cases, the fruit itself is misshapen, becoming kidney-shaped or otherwise distorted. This happens when any one of these happens, and usually to the larger variety of tomatoes. *Temperatures drop below 50 degrees F during flowering and fruit set, resulting in poor pollination. *Excess heat, *Erratic soil moisture *High nitrogen has also been shown to aggravate this disorder. *Too many CATS!! (not really, just a yankin' yer chain Gurl!)

well too many cats sure could be the case here... there was only one mator that i saw like that.. i picked it right away..
